Farewell letter doctor Hermsen

Dear patients,

After almost 23 years, I am very sorry to inform you that I am definitively retiring as your general practitioner. The cerebral hemorrhage has too many consequences, so I am no longer able to be your doctor. We’ve been through a lot together. Unfortunately, I cannot say goodbye to you in person. I would like to thank you very much for the trust you have placed in me. Fare you well!

Over the past year I have been overwhelmed with cards and other expressions of interest and sympathy which you have sent to me in so many. It makes me confused and I thank you from the bottom of my heart for that. It really means a lot to me.
Transferring a GP practice takes time. It is difficult to find new GPs. To ensure that the care for you and other patients continues as usual, I transfer my practice to Huisartsenzorg Delft, founded by the Care Organization First Line (ZEL). This is a support organization of and for general practitioners. This guarantees the future of the practice. And the practice explicitly does not change into a ‘commercial practice’, about which the media write a lot.

What does the transfer mean for you concretely?
– The practice remains located at Beukenlaan 4a, you can reach it via the well-known telephone number.
– Doctor Kuip, Aline Tóth and the other practice staff will continue to work in the practice.
– The website will continue to be used.
– We transfer your patient data to Huisartsenzorg Delft to ensure continuity of care. Your medical data is only visible to the care providers who work in the practice. Do you object to us transferring your medical file? Please inform us of this within four weeks by e-mail to: info@dokterhermsen.nl
